Case 1 History ---- The patient was a 51-year-old female with a past medical history of asthma, hypertension and hypothyroidism who presented on 6/3 with thrombocytopenia (platelets: 7K), anemia (Hgb: 8.3), and elevated LFTs. Her peripheral blood smear showed schistocytes, and ADAMTS13 inhibitor screen was positive with reduced ADAMTS13 activity to 70%). She was diagnosed with thrombotic thrombocytopenic purpura (TTP). The patient was transfused with platelets and scheduled for plasmapheresis. Her platelet count bumped up to 27K. An initial CT on 6/5 showed no acute intracranial findings; however, on 6/6 her platelets again dropped to 4 and she had a seizure which initially abated without medications. She was transferred to the ICU and had repeated seizures, desaturated and became hypotensive. She was found pulseless and ACLS was initiated. However, the patient never recovered and was declared dead on 6/6.
