Fibrin-platelet microvascular thrombus. ---- Comment: Fibrin-platelet thrombi with prominent CNS symptoms was thought consistent with thrombotic thrombocytopenic purpura. An ADAMTS13 assay could not be performed in this 40 year old case, the presence of preserved clotting factors and function and absence of hemolytic uremic symptoms is consistent with a TTP diagnosis rather than HUS or DIC.
