There is marked central vermal granule and Purkinje neuron loss, atrophy and Bergmann gliosis. (H&E) ---- Comment: It is thought that the patient had an injury within a short time of birth or even prior to birth, possibly with prematurity or respiratory issues which left him with a static encephalopathy, sometimes referred to colloquially as “cerebral palsy”. In fact, many of the pathologic features of hypoxia/ischemia in this patient, such as ulegyria, loss of neurons in the basal ganglia, mineralized neurons of the thalamus, central vermal neuron loss and astrocytosis, and widespread white matter pallor and loss are more often seen in neonatal hypoxia/ischemia than as acquired lesions in adults.
