Case 16 History ---- The patient is a 47-year-old woman with seizures who presents with migrating T2-hyperintense lesions involving the left amygdala and right insular cortex, now with nodular enhancement. Operative procedure: Right temporal stereotactic biopsy. ---- 16A1 H&E stained sections show fragments of gray and white matter with small blood vessels with lymphohistiocytic cuffing and angionecrosis. Some vessels also have associated multinucleated giant cells. Scattered vessel-associated macrophages show hemosiderin uptake. The brain parenchyma otherwise shows reactive astrocytosis and microglial activation. (H&E)
